Report filed by the District Magistrate, Pune regarding Suo Moto matter in O.A. No. 130 of 2021 on the death of 18 people, mostly women, in a fire at a chemical factory in village Urawade, taluka Mulshi, district Pune, Maharashtra on June 7, 2021. 

The District Magistrate, Pune being the president, Disaster Management Authority, Pune by order, June 7, 2021 constituted a committee under the chairmanship of SDM Maval to look into the disaster at the factory, SVS Acqua Pvt Ltd. The factory had permission to produce Chlorine Dioxide tablets and powder for water purification by formulation process.

The committee after inspection said that large stock of flammable substances were stored in the place and the actory had not informed the Industry and Health Department about it. The storage of the flammable materials and workplace was the same and there was no final NOC from the Fire Department and no fire extinguisher in the premises. There was also the possibility of production of flammable material other than the product mentioned in the consent letter issued by the Maharashtra Pollution Control Board (MPCB). An FIR has been lodged against the owner of the factory and an offence has been registered against them in Paud police station.
